Income verification with ONE of the following:
- Two most recent and consecutive Pay stubs for each parent
- Proof of WIC benefit (letter of benefits or shopping list)
- Proof that parent is enrolled in Medicaid (letter or card in parent’s name)
- Proof of SNAP benefit (notices or benefits details from DHS)
- Most recent tax return
- Most recent W-2 for each parent
- Proof of Supplemental Security Income (SSI) benefit
- Proof of Temporary Assistance for Needy Families (TANF) enrollment
- Proof the family receives Child Care Assistance Program (CCAP)
